About the service
Our innovative specialist team of diabetes nurses, educators, and admin team support people living with diabetes across the Portsmouth area. We aim to deliver holistic care; empowering individuals who have been diagnosed to self-manage (where possible) and we collaborate with community diabetes services and GP practices.
What we do
As a non-case-holding team, we receive requests for clinical guidance and education.
We run courses for people with a new diagnosis of type 2 diabetes across the Portsmouth area. We offer a 1.5-hour online or a 2-hour face-to-face ‘New to Type 2 Diabetes' course. This will help you to learn about your condition and manage it more effectively. It is also a great opportunity to meet other people with diabetes and raise questions.

Accessing the service
To find out more please contact the Portsmouth Community Diabetes Service by calling 0300 123 3396. You can also email the service at hiowh.ports-diabetesnurses@nhs.net. Healthcare Professionals (HCPs) can also contact us to book onto one of our courses for HCPs.
Please note that we do not accept referrals from patients by email or phone call. You will need to ask your GP or Practice Nurse to send us an official referral.

Our diabetes courses
This course is for anyone who has been diagnosed with type 2 diabetes within the last 12 months. The course covers how to manage type 2 diabetes, including dietary advice, exercise recommendations, foot care, advice on driving and the DVLA, and information on remission.
You can decide to attend the course online or face-to-face.
The online course is a 1.5-hour session delivered by educators from West and North Hampshire.
The face-to-face session is a 2-hour session delivered by diabetes nurses covering the Portsmouth area and will be held in Buckland community centre.
Both courses will allow you to ask questions and connect with other individuals.
How to access these courses
Your GP or Practice Nurse can refer you to the course via their e-referral system.
The Portsmouth Community Diabetes Team offers diabetes education and support to primary care and healthcare professionals working in community settings across Portsmouth.
