Frailty Urgent Community Response (UCR) and Hospital@Home

About the service

We support individuals living with frailty who have acutely decompensated, which could lead to hospital admission including:

  • Falls
  • Immobility
  • Delirium
  • New onset incontinence
  • Medication side effects.

We are not able to provide:

  • Acute emergency care, e.g. for individuals who are injured
  • An initial two-person response (e.g. for individuals who are on the floor)
  • Bridging the care gap 
  • Intervention for individuals whose decompensation is due to drugs and alcohol

End of life care.

What we do

The team works with the patient to:

  • Diagnose and treat the cause of a patient’s acute deterioration
  • Provide a person-centred approach, including future care planning.
  • Support a person at home through the crisis and discharge them back to the care of their GP - usually within 7 to 14 days.
     

Accessing the service

We are only able to take referrals from:

  • Health care professionals (from hospital, GP practice, ambulance services, nurses, and therapists)
  • Social care colleagues.
